At each appointment, patients must bring a complete list of their current medications along with their prescribed pain medication, in the original prescription container.

All new patients will be asked to provide a urine sample for a drug screen. After the initial visit, patients will undergo randomized urine drug screens determined by risk assessment.

No medication will be prescribed on the first visit. The only exception is for those patients currently being treated for opioid use disorder. Benzodiazepines and opioids are not prescribed together. If patients are currently being prescribed these medications, the provider will work to taper benzodiazepines, as this drug class is no longer recommended for long term use.

Minor children are not permitted in the office.

To ensure maximum time with our providers, please arrive at least 15 minutes prior to the scheduled appointment time. An appointment may be rescheduled if patients arrive more than 15 minutes after the scheduled arrival time. If patients are unable to keep their appointment, please call to reschedule.

This depends upon your diagnosis and progress. Some of our patients will have appointments once per week, every 2 weeks or once per month. Your progress depends upon your individual situation, including your adherence to the plan of care, other health concerns, etc.

Please check your insurance benefits for any co-pays, deductibles and out-of-pocket amounts. We offer a self-pay fee schedule to those without insurance.

  • A current, valid government issue photo ID
  • Valid and current Insurance card at every appointment
  • Current list of medications
  • All medications in their original container
  • Form of payment for the first visit.

How do I obtain my medical records from Elite Pain?

Please print, complete and sign the attached form. You may fax it in to us at 417.888.0189 or you may bring the completed form to our clinic.

Please Note:

Medical record requests are processed on a first come, first served basis. We typically have multiple requests at one time and get them out as expeditiously as possible. Please be patient.

Be sure to fill out the form completely. Your request could be delayed if you leave out dates, specific records requested, or other details.

We will contact you when the records are ready to be picked up. Please bring a valid ID with you for verification.

If you are a new patient, you are automatically registered if you provide us with an email address upon scheduling. If you are an existing patient, we can easily set up a patient portal account once an email address is added to your account.

With an online patient account, you don’t have to leave home to keep your health on the right track.
